To give a surgical opinion,from a physician point of view it is quite a difficult task.
But, para-surgically I think this case of SRUS, can be managed with a varti prepared by dipping a sterile cotton gauze with chnageryadi ghrtam or the same can be used as a basti dravya.
Internally, the varanadhi kashayam may be thought of.
W.r.t homeopathy there is a wonderful remedy called Condurango which is indicated in intestianl ulcers.

If this patient came to me i would prefer to put this patient strictly on cabbage juice and coconut water for 5 days and after that i prescribed swarn soot sekhar Ras and Kaamdudha Ras twice a day .

* if this is not treated in this stage then it will converted into piles.

Let us first make a correct diagnosis either it is Solitary Rectal Ulcer Syndrome or Anal fissure. If it is SRUS the treatment is same as that of Grahani if it is fissure then Jatyadi taila, Abhyarishta, Oint Pilex and Triphala guggulu will do
